Odontogenic Keratocyst: Towards a More Conservative Surgical Approach Through Piezo-Surgery

Abstract
Introduction: The odontogenic keratocyst is a developmental cyst of the jaws that can expand and lead to mechanical complications such as pathological fractures. Early diagnosis is essential to prevent these complications and tailor appropriate management.
Materials and Methods: We report the case of a 28-year-old patient presenting with a large mandibular keratocyst, whose progression had eroded the inner cortical plate and thinned the basal border. A complete enucleation was performed following an access osteotomy using a piezotome and deliberate sectioning of the involved dental structures. This approach allowed cleavage without stressing the bone structures, which were significantly weakened by the lesion
Discussion: The keratocyst has a high potential for recurrence, requiring appropriate management and close follow-up. The integration of piezosurgery allows for more precise and atraumatic resection, thereby preserving bone structures and reducing complications.
Conclusion: Early management of the keratocyst is essential to prevent functional consequences. It must be carefully planned and guided by an approach that is as conservative as possible. In this context, piezosurgery represents the gold standard in treatment, allowing for a less invasive and safer intervention.
